Use this form to request Exchange mailboxes for both individual employee use and departmental use. Your request will be processed through the Help Desk ticketing system and will allow you to track its progress.
IMPORTANT for new employee mailbox requests - The employee NetID is available in Banner via the Additional Identification tab on the PPAIDEN, FOAIDEN or SPAIDEN forms. If you do not have access to Banner contact the person who is responsible for your department's HR administration for this information. Administrators who have access to Banner but not to PPAIDEN, FOAIDEN or SPAIDEN can request it using the Banner Account Request form.
Contact HR if a new employee's account cannot be verified to ensure that he/she has an employee status. To find the HR Personnel representative for your department go to www.montana.edu/hr/Contact.htm.
Note: The format for employee addresses will be the employee's first.last followed by @montana.edu.
