IP Request Form

Complete the following form to request a new IP address, to transfer an IP to another device, or to update DNS information.
There is a $50 fee for a new IP address.

A new IP address is required when a device is moved to a new subnet (vlan). Generally this is the case when moving to a new building. In such cases the old IP address may be kept by the department for other use or returned where it will be recycled back into the general inventory.
