MAP Final Performance Review
(Parts 2 & 3 of PPR Form)

Supervisors:
  • Schedule a convenient time for both you and the employee, when you can meet away from distractions and interruptions.
  • Ask the staff member to come to the meeting prepared to discuss the points addressed in previous Checkpoint Review Meetings.
  • You might want to solicit feedback from customers and co-workers.
  • Write a preliminary evaluation based upon your knowledge of the staff member's work habits, accomplishments, and efforts throughout this performance cycle.
  • During the meeting (plan on one hour):
    • Discuss the results achieved in relation to agreed upon goals.
    • Review the Job Success Factors that have been learned or strengthened, and those that could be enhanced.
    • When reviewing the staff member's input, listen to his/her point of view.
  • After the meeting:
    • Finalize the Performance Review by completing Part 2 & 3 of the Performance Planning & Review Form.
    • Both employee and supervisor sign the form and forward to the next supervisory level for final review and signature. Then forward to Human Resources/Personnel Services.
    • Schedule a date for finalizing the next cycle's goals.

Employee:
  • Share the responsibility for the Final Performance Review meeting.
  • Take the time prior to the meeting to review notes and discussion points from previous Checkpoint Review Meetings. Think about how you would answer each of the discussion points.
  • Be prepared for this meeting and engage in a two-way dialogue to help ensure an effective discussion is achieved.
  • Take the opportunity to provide any written comments.
  • Be prepared to discuss your ideas for the next cycle's Performance Plan.
  • Sign Part 3 of the Performance Planning & Review Form. Your signature does not indicate agreement or disagreement with the review; it certifies that a formal performance discussion took place.
