Performance Review Cycle
Reminder: Eligible staff members cannot receive the Achievement Pay if Parts
1A, 1B, 2, and 3 of the Performance Planning & Review Form are not completed on or prior to deadline.
Note: Completed forms must include Dean/Director signatures.
July 2005 (Specific deadlines may vary by campus)
Deadline to develop and complete Part 1A of the Performance Planning & Review Form
for eligible employees.
October 2004 - January 2005
Checkpoint Reviews must be completed and documented in Part 1A of the Performance Planning & Review Form. Number of
required Checkpoint Reviews and specific deadlines may vary by campus.
June 2005
Deadline to conduct Performance Review Parts 1B, 2, and 3 and submit a completed Performance Planning & Review Form to the campus office of Human Resource Services/Personnel
Services. Deadline dates for the form submission established per each campus.
June 30, 2005
Eligible employees receive .5% Achievement Pay.
