To sign up for tutoring you must agree to the following terms:

1. I assume sole responsibility for my final grade.
2. I understand that tutoring does not guarantee a passing grade; hard work on my part in conjunction with tutoring will promote progress in a given course.
3. I will attempt to complete all assignments, read all assigned class material, attend all classes, and make my best effort to complete assigned problems or assignments before coming to my tutoring appointment.
4. I will come to each session prepared, bringing texts, syllabi, assignment sheets, notes and questions.
5. I will meet with my tutor on time for all scheduled appointments. If I know that I am going to be late or will miss an appointment, I will contact my tutor directly to let them know.
6. I will participate actively in all aspects of the tutoring session (asking questions, completing practice problems, etc.).
7. I understand that my tutor will not do my work for me or help me more than I am willing to help myself.
8. I will adhere to my tutoring schedule. My tutoring schedule is considered a commitment, and I will keep it in mind when making any other appointments.
9. I understand that missing two tutoring sessions without notifying the tutor in advance may result in the loss of my tutoring privileges.
10. I will contact the Office of Student Success if I have concerns about my tutor.
11. I agree to perform an evaluation of my tutor(s) at the end of the semester.
12. I understand that failure to comply with these conditions may result in the loss of my tutoring privileges.

I have read and understand the above conditions to receive tutoring through the Office of Student Success. I understand that if I do not follow the guidelines listed above, that I may no longer be able to be part of the SmartyCats Tutoring program.
