MSU Bozeman Events with Alcohol Request Form

Form must be submitted at least three weeks prior to event to ensure adequate notification of all parties.

Please note that per policy 1091.00 Authorization Requirements of the Facilities Use Manual the primary focus of the event cannot be alcohol and all events with alcohol must provide non-alcoholic beverages and food.

I have received and reviewed the Events with Alcohol Requirements and Alcohol Marketing and Risk Management Guidelines. I have reviewed the Alcohol Purchase Policy. I accept the responsibilities outlined. I also acknowledge that MSU will determine whether security is required for the event and will order and change the costs of security to the event organization. I attest that should a Banner Index number be used to purchase alcohol that it is an appropriate use of funds and I have been authorized to utilize funds in this manner.

Approval requests will be automatically submitted to the Director of Auxiliary Services, Assistant Vice President of Finacial Services (only if using University funds), and the Vice President for Student Success.
