Form must be submitted at least three weeks prior to event to ensure adequate notification of all parties.
I have received and reviewed the Events with Alcohol Requirements and Alcohol Marketing and Risk Management Guidelines. I have reviewed the Alcohol Purchase Policy. I accept the responsibilities outlined. I also acknowledge that MSU will determine whether security is required for the event and will order and change the costs of security to the event organization. I attest that should a Banner Index number be used to purchase alcohol that it is an appropriate use of funds and I have been authorized to utilize funds in this manner.
When holding an event at the MSU Foundation/Alumni Center where alcohol is to be served, the MSU User agrees to fully comply with all laws, rules and regulations governing the serving and consumption of alcohol as well as any other laws and regulations governing the event. The MSU Foundation assumes no responsibility for or liability associated with the serving or consumption of alcohol during events held in the MSU Foundation/Alumni Center that are not sponsored by the MSU Foundation.

Approval requests will be automatically submitted to the Director of Auxiliary Services, Assistant Vice President of Finacial Services (only if using University funds), and the Vice President for Student Affairs.
