| In addition to the proposal you will need to fill out and submit a three-page application. Please make sure to provide the following information. Note: the completed application package includes all of the following: |

You must fill this form out completely, including your local address and contact information. Please indicate whether your project is eligible for funding from the INBRE program. If you are not sure, visit the INBRE page on our website or contact Martin Teintze (mteintze@montana.edu). Please make sure to provide all requested information including:
- Are you a U.S. citizen?
- Are you a past participant in USP? If yes, what year(s) and semester(s) did you receive support?
- Are you a full-time undergraduate student? (During the academic year you must be a full-time student to receive support. During the summer you don't need to be full-time, but you need to have been full-time during the previous spring semester and be planning to be full-time during the following fall semester.
- Please provide your current address so that we can contact you in a timely manner. |

| Summary Statement |
| The Summary Statement is not the proposal, but simply a brief description of the intended project. You may choose to type the Summary Statement on a separate page. If this is the case, please indicate that you have done so in the Summary Statement box. |

| Project Schedule |
| This form provides space for your proposed work schedule to justify the proposed salary and/or fee waivers. Please list below the number of hours to be spent on each phase of your project. |

| Budget |
| Your budget should provide information concerning what you will purchase with money, if any, that has been requested for supplies. Check with your faculty sponsor regarding eligibility for money and expenses. |

List each item necessary for the completion of the project, including equipment, supplies, computer services, library, or laboratory fees, and travel expenses. |

Estimate the cost of each item. |
 |
Do not list items that have been or will be purchased prior to the anticipated date of the award letter. |
 |
Expense allowances are usually paid as expenses are incurred. |
 |
Arrangements for payment will be made when the student responds to the award letter. Disbursement of funds is at the discretion of the program director. |
