Certification of Eligibility for Accommodationss
Once disability status has been established, a student will be asked to complete a "Declaration of
Disability and Request for Accommodations and Adjustments" form (see Appendix A). The student
will meet with a DSS staff member to review accommodation requests and to explore other
disability-related needs. Each accommodation request will be given serious consideration, and
requested accommodation, or appropriate alternative accommodations, that are both reasonable and
supported by documentation will be approved and listed on an accommodations approval form. The
student is then issued a DSS certification card, which is presented to instructors when
accommodations (e.g., note taking assistance or extended time on exams) are requested. Copies of
all forms used in the accommodation request and approval process become part of the student's DSS
file.
DSS personnel understand that accommodation needs may change as new challenges arise or if the
impact of the student's impairment changes. Therefore, student are encouraged to check in regularly
with DSS personnel. This regular contact allows staff to monitor the usefulness and appropriateness
of each accommodation .
