Worker Instruction Handouts
At the first visit, each worker will be given a
packet which
include eight handouts. Please refer to them as directed
during the home visits. In addition, workers will receive a
copy of the brochure titled, Understanding Acute Low
Back Problems, by the Agency for Health Care Policy and
Research. The worker handouts include:
Low back pain information
card
Using medications
effectively
Safe lifting and moving
Working effectively
with your health care providers
Back treatments and
devices: What works and
what doesn't
Promoting a healing
environment: Rest, nutrition, and stress management
Keep moving: Exercise
and fitness during recovery
Maintaining a fitness
program
Snow Shoveling
with Care
In addition to the handouts, two other items
are included:
Health goal form
(form for contracting with worker)
Chart form (to
document each home visit)
