Employee Performance Evaluations
Annual Performance Evaluations are due for submission by June 30, 2021 for classified and non-faculty contract employees. To streamline the process amidst the COVID-19 situation, we have developed a digital-friendly "EZ" form, below. Note: Older versions of evaluation forms will also be accepted.

Form - All Employees
A new EZ form is now available and is suitable for all employee categories (staff and non-faculty contract professionals). This is the recommended form.
