Telephone & Network Service Requests [TNSR]

Please use theTNSR form below to request any of the following services:

  • Activate jack - data or voice
  • Directory - additions, updates, deletions
  • Disconnect a service
  • Move a phone to different location
  • New phone service
  • Order new phones or accessories
  • Upgrade/change an existing service
    long-distance access, phone display name, etc.
  • Voicemail - add or update

NOTE: If your request is to order, move, or disconnect the phones or phone services for an entire office or department please contact the Telephone Office at 994-5050 before completing this form.
