- In Absentia
- Please download the form and mail the form with $30 payment to the Registrar's Office.
- Minor Application
- Students who have not yet filled out the form can email their completed minor application form along with advisor approval to [email protected] and will be forwarded to the appropriate auditor.
- Name Change
- Please send completed name change form and pictures of new DL, SSN, and official document (divorce degree, marriage license) to [email protected]
