CO-OP Application Form Upload

Please upload the following materials:

1. A one page essay

Include information about your financial need, your academic background and preparation, and how involvement in CO-OP might impact your MSU career. Also include a paragraph of interest addressing your personal and professional goals and why you selected Montana State University. Please revise and proofread your statement carefully.

2. Documentation of enrollment or lineage

All descended and enrolled Native American students are eligible for the CO-OP. Please provide your tribal enrollment or lineage paperwork. Some scholarships require the BIA-4432 form.
