Note-Taking Request
Please submit this online form to request a notetaker for your class(es). You may also make a note-taking request in person at the Disability Services office in Romney Hall room 137. Lab, clinical, and recitation sections are not eligible for notetaking assistance. For more information on receiving the notes after a note-taker has been found, please refer to the Policies and Procedures page under Approved Accommodation Procedures.
