Used to give permission for the Student Account's Office, Residence Life, and the Office of Financial Aid Services at MSU to release financial information, in accordance with MSU's FERPA policy. |
| Computer Purchase Policy |
|
|
The Higher Education Amendments of 1998
included a provision that allows an
institution, on a case-by-case basis, to
include in a student's cost of attendance
(COA) the documented cost of a personal
computer. Allowing inclusion of the cost is
not based on any particularprogram of study
or program requirement. The following
policies and procedures will be in effect
for Montana State University Bozeman
undergraduate and graduate students. |
| Consortium Agreement |
| |
A
consortium agreement enables you to receive financial aid while
concurrently enrolled for courses at Montana State University-Bozeman
and another institution. |
| Dependent Care |
| |
Please
provide information for each dependent living with you for
whom you will pay dependent care expenses. Paid receipts, canceled
checks, billings and/or a written statement of estimated cost
from childcare agency must be attached. Dependent care expenses
may only be considered for only periods that you are enrolled
as a student at Montana State University. |
| Faculty & Staff Fee Waiver Authorization |
|
Retroactive
fee waivers will not be honored. Employing department must
be an entity of any unit of Montana State university or University
of Montana system. A separate form must be completed for each
semester for which a faculty/staff fee waiver is being requested. |

Due to changes in federal loan limits, you may be eligible for up to an additional $2,000.00 in Unsubsidized Stafford Loan funds. If you would like the Office of Financial Aid Services to review your eligibility for these additional funds, please submit this form. |

Quality Assurance Verification

Financial
aid is only offered through your participation in an approved
Montana State University study abroad program. If you enroll
in a study abroad program sponsored by other organizations,
colleges or educational institutions, or enroll directly in
a foreign institution, you should contact the school or program
to learn about financial resources available. |
| Study Abroad Budget |
|
This
evaluation of estimated expenses is for those students who
are applying for Financial Aid. Fill out the Estimated Expenses
yourself to the best of your ability and then bring the form
to the Office of International Programs for signature. Then
take one copy of the completed form, along with any required
documentation, to the Financial Aid Office and provide one
copy to the Office of International Programs. |
| Study Abroad Course Approval |
|
Please
meet with your academic advisor to fill out this form. Complete
the backside for the Office of Financial Aid. Turn in at the
Office of International Studies when complete. |
| Summer Supplemental Form - 2008 |
|
The priority consideration date for financial assistance for the 2008 Summer Session is March 31, 2008. Please complete this Supplemental Application Form and return it to the Office of Financial Aid Services. The last date to apply for summer aid is June 30, 2008. In requesting summer session assistance, you will be considered for need-based grants and scholarships according to enrollment status, need, and available funds. |
